Codes vary with time and location. Still the norm is to run a service line to your house and set a meter base. Wiring from your breaker panel connects to meter base. There should be a #8 ground, in one unbroken piece, run from your panel to meter base, to ground rod(s), to utility pipe. It gets bonded to all those locations as well as your breaker panel. Inside your panel the neutral and ground should be bonded together too.
What you don't want are a bunch of individual wires going to a bunch of different grounds all over the place. Establish one and only one ground and bond everything to it only. In my local we call for three 5/8" X 8' copper coated ground rods in a triangle pattern. I think they now want a #6 green insulated conductor.
