Chris,If I am not mistaken, I have seen practically the same exact topology used on the top-line AudioCraft (Ultracraft) phono stage, circa 1994.
The reason why AC went for this topology was that they wanted to get a balanced output, but using only a single (non-complementary) voltage amplifier front end, and with global NFB.
The basic topology was dual-differential, but rather than using a current-mirror to combine the outputs of the second-stage differential and drive the output stage per normal practice, they used active loading for the second-stage, and used each "tail" of the second-stage differential to drive its own set of output devices.
One output stage was used for the + part of the balanced signal output, and the other stage was used for the - part. But the corrective global NFB was only derived from one of the output stages - the other output stage was run without global NFB.
At least that's what I remember. In other words, the reasons were different, but the final topology was quite similar to that shown in the Altman article.
