I found a place

I found a local glass place (The Glass Doctor) to make a replacement and drill the holes. The estimate is $45.00. They won't be able to bevel the outside edge but maybe I can do something with a Dremmel when I get it back. It's two 1/8" glass pieces laminated together with two holes.

Since I've seen several of the TU-9900 for sale with the same crack I'm assuming the Tuning Dial is stressing and breaking the glass, probably in shippment. The factory holes are 3/8" so I'm having them enlarge the holes to 1/2". The dials will easily cover these and there will be a little more room for movement in shipping.

I might suggest to others that the tuning dial be adjusted out a bit so there is some gap between the dial and the glass behind it before it is shipped. You could even remove the dial for shipping to be safe. I suspect the large dial is getting stressed or hit in shipment, breaking the glass behind it.
