Where does the 12V (13.8V) come from?

That designation normally indicated it is designed to charge or replace a 12v car-type batery (eg for a Ham radio).

13.8V is the voltage required to charge a battery.

You may find that without a load it will go to a higher voltage. Connect a 12V car bulb across it and see what you get. If it is still high, then something is blown, possibly the regulator, but impossible to say without measuring things.
