Richard, your preamp is fine.The phase test on your record is to detect a polarity error on one channel relative to the other. The notes for your record appear to be unclear, but what they want you to listen for is whether the voice is in focus when it is stated to be "in phase" and out of focus when it is stated to be "out of phase." The preamp polarity switch has no effect on focus during the "out of phase" segment, because one channel or the other is always inverted.
If you heard the voice as being in focus during the "out of phase" segment, and out of focus during the "in phase" segment, then you would have a wiring error somewhere that inverts the polarity (equivalent to a 180 degree phase error) of one channel. Since you did not, then your wiring is also correct.
If you can hear a difference in voice quality (timbre) or position when you throw the polarity switch during the "in phase" segment, then please let us know.
