"What audible difference is there ..."
Does 'the EXTREME premium price get you a significantly more natural sound?'
'is the return too diminished in comparison to cost?'
Good questions that are hard to answer. The answers regarding improved sound quality and cost effectiveness are both very system dependent. What sounds rich and ultra transparent in one system may sound thin and threadbare in another. I think that a person could use the AN levels as a guide. Combinations in a given level work well. AN components in a given level work well together (good system synergy). Straying more than a grade would likely detract from this synergy. For example, placing level 5 transformers in a level 2 system would likely result in only a modest improvement in sound quality. OTOH, the same upgrade in a level 4 system would likely produce a much greater improvement. Some apparent upgrades might actually result in some loss of sound quality.
There is a bewildering array of transformer upgrades available for the transformer coupled dacs. They have 3 transformer coupled interfaces and there are options for each of these. Arguably, upgrades would be most effective from the input forward. Judging by posts here, upgrades of the digital input and I/V transformers are frequently overlooked or seen as an afterthought.
Of course all the current offerings will be eclipsed by the new Infinity+1 series transformers featuring liquid helium cooled superconductors and a scintered kryptonite core.

Just my $0.02
YMMV
