I bought them in one of the hi-fi stores in Tokyo, maybe it was Audio Union, but they sell this stuff everywhere. See the link for Yodobashi Camera below.
I've just realized that I got the SH-22, which is A5 size, whereas the SH-21 is A4 size (twice the size of SH-22). It cost about $30 as you can see at Yodobashi.
