If it's really a tape out then you should have no gain control. (The tape out bypasses the preamp's gain control). I wonder how you can change the volume? But, generally speaking, the tape, or monitor out, is what you should hook up your headphone amp to. But I am surprized there is so little voltage coming from your CD source. It should be loud (2-5V).
The alternative is to bypass the receiver and connect the headphone amp directly to the CD source.
Does the CD source go into an input rca on the receiver or to the tape in ?
Try it with both. Normally it would just go to a regular input.

Follow Ups:
I finally got it figured out with a little help from a tech at Marantz. You use one of the Zone outputs without turning on Zone 1 or 2 in the Amp Assign section of the menu. You leave Amp Assign in Main Mode. I was turning on Zone 2 in the menu and it, in turn, was turning on the rear 7.1 surround speakers and bypassing the Zone 2 output. Damn, this is still confusing.

Glad to hear the problem is solved. I have just 2 channels and 2 speakers. Though I remember from much earlier days a Yamaha receiver I had that A and B, and I was always having to fiddle with those switches. I think if I ever needed 4 channels, I'd get 4 identical mono amps run put my digital source into 1 set of inputs and though 2 sets of outputs in my preamp into each monoblock!

I know what you mean. I wish I had a separate room for strictly two channel, unfortunately I don't. Ah well, maybe one day......
