Typically a device with a transformer in it that has one primary winding and 2 secondary winding with a turns ratio that gives you 60 volts to ground from each of the secondary windings or 120volt line to line. Balanced or isolated power is not referenced to ground and is a little dangerous if used improperly (like for lighting) and technically is in violation of the National Electrical Code in a residential application.
I have my headphone rig plugged into a medical/hospital grade, balanced/isolated power filter which is little more than a transformer described above connected to a couple of receptacles in a metal box with a cord set and power switch. It does nothing for the sound of my headphone system really. I can actually hear noise from the dimmer of a floor lamp that is plugged into the same recpt ckt that the iso box is plugged into, so it's is not doing anything to cancel that particular type of noise on the line. YMMV.
