This is often indicating a need for better PS rails and ground runs, meaning that the player's rails and ground are not low enough in impedance. Adding bypass capacitor does not always help, and in some cases of a really lousy set of rails/ground, can actually make things worse.Disconnecting the opriginal power supply rails and ground, and running a "home run" set fronm the power supply regulator terminals can often help a lot here, as long as suitable bypass caps are added at the op-amp pins for this set of "home run" rails.
Also be aware that many of the op-amps originally designed for video use not only require a 75 ohm output termination for fulll rated stability, but also have far greater requirements for good low Z rails.
Finally, I have heard of problems with more recent production runs of OPA-2604 IC's that had 'hot' higher gain than design transistors in them, and a consequent tendency toward instability. The cure is lot's of gain and a good set of rails.
