Probably the 417A getting upset...

...remember that the 417A will oscillate like a... well, oscillate really badly if given half a chance. It doesn't like much more than a 1K grid resistor (I use 10K max) and likes a grid stopper as well. Oscillation can create audible hiss. As you increase the volume setting you increase the value of the "grid resistor" as seen by the 417A, so the symptoms make sense.
