Rear-loaded horns allow you to get better bass response from high fs low Qts woofers than a direct radiating VB alignment can deliver. They are nothing like 4th order VB as they aren't bandpass. They are no more sensitive than the same driver in a VB, they just have somewhat better bass extension. The concept of rear-loaded horns for PA was rendered obsolete with the advent of the pro-sound subwoofer. Rear-loaders typically were used in two way systems, along with HF horns; modern high power systems are typically front-loaded midbass horns/HF horns crossing over at 80 to 100 Hz to horn loaded subs below.
