You could use a total of three pieces of heatshrink
One piece to cover the length of the old lead upto the point where you will solder(shrink it)
Another piece to cover most of the new lead up to the point where you will solder it on the old lead(shrink it)
The "middle" piece at this point need to be very short and should fit over one of the two already shrunk pieces. Solder , cover the joint with the third piece , shrink it and you are done. :)
I call it "telescopic heatshrink installation"
