I agree with Bill. Jensen transformers are the solution. They have a good assortment of isolators/converters/etc.http://www.jensen-transformers.com/iso_aud.html
I've seen the CI-2RR at Markertek.com for a decent price. This isn't exactly what you want, but you could remove the transformer/circuit assembly and reconfigure it for balanced/unbalanced conversion if you like.
