I had star grounded everything, that is to say all the ground leads tied together at one spot... more or less. By more or less I mean within 20mm or less. That was not, however, sufficient to avoid a classic ground loop. The output current got into the input current though the rather wide star ground.To fix that, all that was required was to take the ground leads from the attenuator and RCA input jack and, rather than connecting them at star ground, solder them to the base of the input resistor. End of problem.
For those reading this, I suggest a "binary star ground". Input signals together at one point, output and power at another point, joined together by a minimum lenghth of high conductivity bus wire.
I have learnt my lesson: probably ok is not good enough where grounding and opamps are concerned.
