1. The latter is preferred. Digital sources pass RFI through the ground side. Just be sure the contacts track where the hot side makes contact last- otherwise you may get transient "spikes" (or maybe loud hum if you move selector slowly) out the speakers as you switch components.2. Depends if you get the problem mentioned in (1), if you don't have the problem, the shunt resistors may not be necessary.
3. Experiment. You may or may not prefer one way to another. Although you may also want to try separate left and right volume controls, and omit the extra transformer. (This is how the ASL Passive TI is set up.)
