Passive preamp grounding scheme

I am designing a passive line stage using transformer attenuators. The issue of grounding comes up and I would like to seek the opinion of inmates here.

1. Should the grounds of the input RCA jacks all be connected together to ground, or should I use a double pole rotary switch to switch both the signal and the ground of each RCA?

2. Should the signal lead of unused RCA jacks be shunted to ground via a resistor?

3. I am going to have a transformer as main attenuator, cascading to a second one for balance control. One side of the secondary of the front transformer will be connected to the corresponding side of the primary of the balance control transformer. Should this also be grounded or left floating?
