RE Q1. What you have posted does not allow a definitive analysis of the input circuit. Only one side of the balanced input circuit is defined, that for balanced input pin 3. It uses onr half of the dual op-amp. It would not be a wholly unwarranted assumption to conjecture that the other ahlf of the dual op-amp is used for pin 2.

RE Q2 & 3. This is a 5532 type op-amp, with the DD package a surface mount type I believe. An OPA-2604 (also available in asurface mount package) would be an excellent drop-in replacement, as it has the drivec apability of the 5532, and ahs less propensity to oscillate than many others would.
