What you have described, "Both neutral/white and both live/blk + ground - connected.", is a common method in older homes and even still done by some electrical contractors today. The device is used as a junction to continue the romex wiring to the next outlet, daisy-chain. Only the equipment ground wire must be jointed and then pigtailed out to connect to the receptacle ground terminal. For a single branch circuit this method meets code to this day. One thing to remember about NEC code, it is bare minimum accepted.How many outlets are on this circuit? On the old recept you pulled out was the conductors wrapped around the terminal screws or stabbed in the back of the receptacle?
