Something similar happened to me after I installed a three prong plug.
Turns out that, when cutting the hole in the chassis for the EIC plug, some metal filings got into the amp and shorted it out. I fixed it by turning the amp upside down and shaking it! Sure enough, a bunch of filings fell out onto my workbench. Amp has worked fine ever since.
