One other idea.In making your mods, you somehow messed up the ground and ended up with -3 v DC on the ground. In other words, "ground" is no longer grounded. That could give you what appears to be +3 v DC on the output jacks, even though it's not present there.
Especially if you made power supply mods, check those carefully for shorts, missing connections, or incorrect connections.
