Yes, that is the answer - one modulation within another modulation.

The suppressed subcarrier phase is directly linked to the phase of the transmitted 19kHz pilot. The 38kHz signal that creates the L-R signal, usually a square wave (or a sine wave) is created by a PLL that phase locks onto the transmitted 19kHz signal. Designing and understanding phase locked loops (PLL) is a study unto itself. That may be another part of the whole scheme that is not obvious.
