Mackie 1202 -- may not be the cheapest, but I know it can do almost all this, and is fairly decently priced. It is a little kludgy -- it's got two stereo busses (they snuck in a second one, which they call 3 and 4), so if you pan two of the mics hard left and hard right, and then route two more hard left and hard right on the ALT34 bus, you can do four separate mics on 4 separate channels. You might be able to use one of the aux sends four the 5th channel, but I think at this point you're pushing it.

The third bullet item you can also do, but not at the same time.

If you can find another mixer that has (a) the inputs you want, and (b) inserts on all those inputs, then you can split the insert to an amp, and redirect the other half of the split back to the insert return.
