At the speaker, the two phases are combined. The speaker sees exactly what it would see with any other amp. For example, in a push-pull amp, the two phases are combined at the primary of the output transformer. There's really no ground reference and there's really no ground reference at the secondary of an output transformer. Some OTLs are ground referenced and not balanced as I'm sure you know. These OTL amps do have a ground reference at their output. I hope all this makes some kind of sense.
