Mystery Headphone Device (by Moth?)

I inherited this piece. As you can see in the pics, it seems to be designed to plug into speaker-level banana jacks, and then give headphone output.
But look at those tiny resistors!
Can anyone out there tell what this is definitely for? Limits of use?
Also, that looks like the old Moth logo, but I can't find a record of this device anywhere on the net, so I don't really know who made this.
