A transistor that frequently blew out on my Quatres (and I believe a problem with the breed)was a MDS-60. It was not a TO-220 case however. That transistor was always unreliable and there are more robust substitutes available. What is the designator for the transistor on the silk screen on the PCB? Is it Q10? I forgot what I replaced these with, but dig thorugh my old notes if you check on the label for that transistor. If it's Q10, there should also be a TO-92 case transistor (MPSA93) right next to it if I remember right.
Also, a nice mod for those amps is to replace the quasi-complementary output stage with a full complementary output and more modern transistors. It's not too difficult and I could give you details if you;re interested.
Good luck -- I was very fond of my Quatres. Unfortunately all that remain are the cases, heat sinks, and the transformers.
