Thanks for all the suggestions. I took out the 5 volt regulators and connect the raw voltage directly to OPA2604. Then added some more cpacitor at the power supply. Since I was there, I also increased the bypass capacitors to the D/A convert.I measured the power supplies to the OP amp (+9,-10). They were horrible with about 90 mV residual A/C signal. However, OPA2604 wakes up. It was a big difference. I hook it up with my rebuilt Harmon Kardon 6V6 amp. The sound was incredible.
One of these days, I will add John Curl's suggestion with a simple transistor filter. I will also try to match all the resistors around the OP Amp. Since the CDP sounds so good now. I am not sure what to expect for those changes.
