You could use the Bob Crump Asylum cord recipe with a few variations.
On your old gear you would not want to attach the ground wire or the drain wire from the cables shield to your amp. So you could remove the outer jacket and foil shield and cut the drain wire and ground wire about 8" from the equipment end of the wire. The 8" dimension is arbitrary and may be longer or shorter depending on the distance you have to go from the strain relief to the point where the cord will be soldered in place. Now cover about 3" of the cable with glue lined heat shrink at the point that you began striping the cable, about 1 1/2" over the jacket and an 1 1/2" over the remaining pos and neg wires. Put the pos and neg wires through the strain relief and let it clamp down on the portion of the wires that are covered by the heat shrink. Twist the two wires together and solder the ends of the pos and neg to the amp.
On the other end attach the pos, neg, drain wire and ground to the male plug as usual.
You could save your self some time hunting for parts by contacting DIYcable .com and buy the Asylum cord kit, less the IEC plug or get them to make you one as described above.
