Here in NYC, it's been code for many decades. Elsewhere, it was done frequently for a variety of reasons. Old wiring is frequently done with BX armored cable. In such systems, the armor acts as the 3rd wire. If your residence is wired with BX, simply replacing the 2 prong outlets with modern 3 prong outlets is all that's necessary. I changed several outlets in my pre WW2 apartment.
