As EdG notes, the GAIN of the non-inverting op-amp is determined by the resiostor ratio. However, 12 dB is not the GAIN, but a log representation of it.Take the gain number, apply the LOG function to it, and multiply by 20 to get dB from GAIN.
RE the 1 uF cap, I would leave it in there for protection from DC on the input, but change it to a good PP cap.
