I would check out the various manufacturers' websites, because they publish application notes complete with schematics for all sorts of circuits: high and low-pass filters, phono preamps, etc. These could very easily be combined to make a crossover.
Some op-amps to try: Burr-Brown OPA2604 and OPA2134, Linear Technology LT1056.
