Hi Mike,you can't have both in one stage. If you have too much noise and offset, cut the gain in two stage. Then, the offset comes normally from the first stage. Single package op-amp offer offset trim pins where you can connect a 100K trim-pot for nulling. The dual package is a comprise for offset. The OPA2604 have greater offset and noise than the OPA2134. The other solution is to use a dual by channel and using cap. coupling between the two.
