Too Much Coffee.....

I don't know what's the *biggest* cause, but the following are contributors.....

- Dirty or scratched media.
- Poor laser alignment/function in transport
- The media itself
- Noise in the digital chain
- Poor bandwidth in digital transmission
- EMI and RFI present
- Power line noise
- Unstable mains power
- Impedance mismatches in electrical digital signal transmissions
- Electrical digital cables with mis-matched characteristic impedance
- Poor digital transmitter design in transport
- Poor digital receiver design in DAC
- The use of intermediate processors (if the transport jitter is already low)
- The improper use of RFI reduction tweaks
- Long digital cable transmissions, especially if unbalanced or optical
